What do I do if I become legally separated or divorced?
  • Remove dependent(s) who are no longer eligible from insurance coverage within 31 days of the dissolution of your marriage/legal separation. You can remove dependent(s) by logging into your CIS profile and entering an enrollment change. You will need to upload supporting documentation. Dependent(s) removed from coverage will receive an offer to continue insurance (COBRA) from CIS.
  • Update your PERS beneficiary designations, W4 information and CIS beneficiary designations, if desired.
